    Limited asymptomatic carriage of Pneumocystis jiroveci in human immunodeficiency virus-infected patients

    Forty-seven bronchoalveolar lavage fluid samples from 16 human immunodeficiency virus (HIV)-infected patients were used to test the latency model of Pneumocystis infection in the human host. Identification of DNA sequence polymorphisms at 4 independent loci were used to genotype Pneumocystis jiroveci from the 35 samples that contained detectable P. jiroveci DNA. Eighteen of those 35 samples came from patients who did not have Pneumocystis pneumonia (PCP) and had confirmed alternative diagnoses. Seven patients had asymptomatic carriage of P. jiroveci over periods of less than or equal to9.5 months after an episode of PCP, and in all 7 cases, a change in genotype from that in the original episode of PCP was observed. The absence of P. jiroveci DNA in one-fourth of the 47 samples and the observed changes in genotype during asymptomatic carriage do not support the latency model of infection. Asymptomatic carriage in HIV-infected patients may play a role in transmission of P. jiroveci and may even supply a reservoir for future infections

    Involuntary civil commitment for substance use disorders in puerto rico: Neglected rights violations and implications for legal reform

    Laws facilitating the involuntary civil commitment (ICC) of people with substance use disorders vary considerably internationally and across the United States. Puerto Rico, a colonial territory of the United States since 1898, currently harbors the most punitive ICC legislation in the country. It is the only place in the United States where self-sufficient adults who pose no grave danger to themselves or others can be involuntarily committed to restrictive residential facilities for over a year at a time without ever being assessed by a health care professional. The involuntary commitment of otherwise-able citizensā€”many of whom have never been diagnosed with a substance use disorderā€”continues to be ignored nationally and internationally. In this paper, we specify how Puerto Rican ICC law and procedures systematically violate rights and liberties that are supposed to be guaranteed by Puerto Ricoā€™s Mental Health Act, the US Federal Supreme Court, and the Universal Declaration of Human Rights. To ensure that Puerto Ricoā€™s ICC procedures conform to prevailing local, national, and international standards, we propose a series of legislative reforms. Finally, we highlight the importance of addressing the preponderance of poorly constructed ICC laws both within the United States and internationally

    Chasing the 400 kyr pacing of deep-marine sandy submarine fans: Middle Eocene Ainsa Basin, Spanish Pyrenees

    In an attempt to understand the relative importance of climate and tectonics in modulating coarse-grained sediment flux to a tectonically active basin during what many researchers believe to be a greenhouse period, we have studied the Middle Eocene deep-marine AĆ­nsa Basin, Spanish Pyrenees. We use orbital tuning of many spectral gamma-ray-logged fine-grained siliciclastic sections, already shown to contain Milankovitch frequencies, in conjunction with a new high-resolution palaeomagnetic study through the basin sediments, to identify polarity reversals in the basin as anchor points to allow the conversion of a depth-stratigraphy to a chronostratigraphy. We use these data, in conjunction with a new age model incorporating new biostratigraphic data, to pace the development of the deep-marine sandy submarine fans over c. 8 million years. Timing for the sandy submarine fans shows that, unlike for the fine-grained interfan sediments, coarse-grained delivery to the basin was more complex. Approximately 72% of the sandy fans are potentially coincident with the long-eccentricity (400 kyr) minima and, therefore, potentially recording changing climate. The stratigraphic position of some sandy fans is at variance with this, specifically those that likely coincide with a period of known increased tectonic activity within the AĆ­nsa Basin, which we propose represents the time when the basin was converted into a thrust-top basin (Gavarnie thrust sheet), presumably associated with rapid uplift and redeposition of coarse clastics into deep-marine environments. We also identify sub-Milankovitch climate signals such as the c. 41.5ā€…Ma Late Lutetian Thermal Maximum. This study demonstrates the complex nature of drivers on deep-marine sandy fans in a tectonically active basin over c. 8ā€…Myr. Findings of this study suggest that, even during greenhouse periods, sandy submarine fans are more likely linked with times of eccentricity minima and climate change, broadly consistent with the concept of lowstand fans. However, hysteresis effects in orogenic processes of mountain uplift, erosion and delivery of coarse siliciclastics via fluvial systems to coastal (deltaic) and shallow-marine environments likely contributed to the complex signals that we recognize, including the 2ā€“3ā€…Myr time gap between the onset of deep-marine fine-grained sediments in the early development of the AĆ­nsa Basin and the arrival of the first sandy fans

    Dedifferentiation of neurons precedes tumor gormation in lola mutants

    The ability to reprogram differentiated cells into a pluripotent state has revealed that the differentiated state is plastic and reversible. It is evident, therefore, that mechanisms must be in place to maintain cells in a differentiated state. Transcription factors that specify neuronal characteristics have been well studied, but less is known about the mechanisms that prevent neurons from dedifferentiating to a multipotent, stem cell-like state. Here, we identify Lola as a transcription factor that is required to maintain neurons in a differentiated state. We show that Lola represses neural stem cell genes and cell-cycle genes in postmitotic neurons. In lola mutants, neurons dedifferentiate, turn on neural stem cell genes, and begin to divide, forming tumors. Thus, neurons rather than stem cells or intermediate progenitors are the tumor-initiating cells in lola mutants
